➠ Get a Library Card?

Your first Aztec Public Library card is FREE and gives access to Aztec Public Library and Bloomfield Public Library.

	➠ Library Card Application (Print, complete form, and bring into the Library.)
	Show valid photo identification with your current mailing address. Can be one of the following: • Driver's license issued by a state in the U.S. • Photo ID issued by a state in the U.S. • United States Military ID Card • United States Passport • Matricula Consular Card issued by Mexico
	If your photo identification does not show current mailing address, another piece of documentation will be necessary to establish your address. This can be ONE of the following: • a recent piece of mail with your name and address; • a rental agreement showing name and mailing address; • a vehicle registration or insurance document with your name and current mailing address; • any other recent documentation showing your name and mailing address.
	Applicants 17 years and under must be accompanied by a parent or guardian with a valid library card in order, to obtain their own card. These cards can be issued at the same time.
	A replacement Library Card is $2.00

	Patron must present a library card in good standing to check out material.
	Up to 20 items may be borrowed on one card.
	Books check out for 3 weeks.
	DVD movies are limited to 5 per card for one week loan.
